Care plans and goals
Goals and plans workers actually see
Care plans in Autonomy Care bring together each participant's goals, a support plan, risk and safety plans, and a short care summary workers read before a shift. Progress against goals is recorded with dates and can be shared with family. The support plan is a reviewed form with an Easy Read version.

How are goals tracked?
Each participant has goals. Workers and coordinators add dated progress, and you can share each update with the family in the Family portal.
What's in the support plan?
About me, my goals, how I like to be supported, risks and safeguards, and health. It's reviewed every 12 months, has an Easy Read version, and can be shared with the participant. Related templates cover risk assessment, emergency evacuation, a one-page profile, communication and safeguarding.
What do workers see?
Before the shift, a care summary: the latest support plan, profiles, goals, alerts and current medicines, on the phone.
What it does and doesn't do
It does

- A reviewed support plan with an Easy Read version
- A care summary before each shift
- Plan review reminders
It doesn't
- Doesn't create shift tasks from the support plan by itself; coordinators set them
- Isn't a clinical care planning system

Participant management
Participant management keeps everything about a person in one record: their NDIS plan and funded services, goals, contacts, plan manager, support coordinator and support requirements. Rostering, progress notes, billing and the Family portal all read from that record, so your teams don't keep separate copies that drift apart.
Progress notes
A progress note records what support was delivered, what was observed and how it relates to the participant's goals. In Autonomy Care, workers write notes on the shift with short prompts, quick notes and dictation. The note coach checks each note against plain rules and current goals, and Signals flag missing notes before invoicing.
Family portal
The Family portal gives participants and their chosen contacts a secure login. With view access, family members see upcoming and past supports, goals and feedback. With full access, such as a nominee, they also see plans and budgets, the support team, shared documents to sign, messages and support change requests. Providers control each contact's access.
Document Centre
The Document Centre holds your forms, registers and policies. Autonomy Care includes a library of 71 ready-made forms you can adopt and change, template versions with approval and review dates, form records with signatures and PDF export, registers, policy acknowledgements with reminders, and a document control register that shows what needs attention.
