Referrals

Referrals that become participant records

Referrals are how new participants reach you. Autonomy Care gives you public referral links to share with support coordinators, families and health professionals. Each referral arrives in an inbox where you mark it contacted, converted or declined. Converting it fills in the new participant record, and declined referrals are deleted after 90 days.

What does the referral form ask?

The person's details, NDIS number and plan management type, services requested, support and communication needs, who is referring, and consent. It has a bot check and rate limits.

What happens when a referral is converted?

The new participant form is pre-filled from the referral, and the referral stays linked to the participant's record. You can also send a quote to a referral before converting.

How long are referrals kept?

Declined referrals are deleted automatically after 90 days.

What it does and doesn't do

It does

  • Gives you several public referral links
  • Collects referrals with consent and a bot check
  • Pre-fills participant records on conversion
  • Deletes declined referrals after 90 days

It doesn't

  • Isn't a public provider directory or marketplace
  • Doesn't accept referrals from the NDIA automatically

Frequently asked questions

Can we have different links for different referrers?

Yes. Create and label as many links as you need, and switch them off or replace them at any time.

Will referral emails contain personal details?

No. The notification says a referral has arrived. You read the details after signing in.

Can we tell the referrer why we declined?

Yes. Declining can email the reason to the referrer.

How do you stop spam referrals?

The form uses a Cloudflare Turnstile check and rate limits.

How it works

  1. Share a link

Create referral links for different sources, label them, and switch them off or replace them at any time.

  2. Receive

Referrers complete a form with the person's details, services and needs, and consent. You get an email without personal details.

  3. Triage

Mark each referral new, contacted, converted or declined. Declining can email the reason to the referrer.

  4. Convert

Converting pre-fills the new participant record and links the referral to it.

Who uses it

  • Intake and enquiries staff

  • Support coordinators (as referrers)

  • Families and health professionals (as referrers)

  • Quotes and service agreements

A service agreement sets out the supports, prices and terms agreed with a participant. In Autonomy Care you build a quote from support items, checked against price limits, send it as a PDF and keep versions. When the participant accepts, the quote lines go into a draft service agreement that you complete and send for e-signature.

  • Family portal

The Family portal gives participants and their chosen contacts a secure login. With view access, family members see upcoming and past supports, goals and feedback. With full access, such as a nominee, they also see plans and budgets, the support team, shared documents to sign, messages and support change requests. Providers control each contact's access.
