Medication and MAR

Medication records and administration (MAR)

Medication management keeps each participant's medication records and a medication administration record (MAR). In Autonomy Care, workers on shift record each dose as given, self-administered, refused, withheld, missed or not required. PRN limits and witness checks are enforced, late and missed doses raise alerts, and refused, withheld or missed doses are escalated.

Part of Rostering and service delivery. Used by SIL providers.

Does it handle PRN medication?

Yes. Each PRN medicine records the indication, the maximum doses in 24 hours and the minimum interval, and doses outside those limits are blocked. Workers record the reason and later whether it worked.

Is there a missed-dose alert?

Yes. Every 5 minutes, scheduled doses with nothing recorded are checked: late at 60 minutes and missed at 3 hours. Alerts go to workers on shift with the participant and to staff who manage medication.

Who can record a dose?

A worker assigned to the participant, on a current shift with them, whose role has the "administer medication" permission. Workers can record doses from the last 24 hours or later today.

For high-risk and Schedule 8 medicines, a second staff member witnesses the dose by signing in with their own email and password on the same device.

Can an incident be raised from a medication error?

Refused, withheld and missed doses recorded by a worker are escalated to the house lead and medication admins. A second refusal in a row, or a refusal of a Schedule 8 or high-risk medicine, is escalated as high priority with a prompt to report an incident.

What does the worker see in the app?

Doses due now, upcoming doses and PRN medicines for the participants on their shift. The worker records the outcome and the time given, with the double check and witness steps where they apply. "Not required" is for scheduled doses only and needs a reason.

After a refused, withheld or missed dose, the app says who was told and offers Call on call and Call 000. Recording a dose needs a connection; doses aren't saved offline. Stock counts are recorded on the web.

What it does and doesn't do

It does

  • Keeps medication records and a MAR chart per participant
  • Records six outcomes: given, self-administered, refused, withheld, missed and not required
  • Enforces PRN limits and blocks duplicate recording
  • Requires a witness for high-risk and Schedule 8 medicines
  • Alerts on late and missed doses
  • Escalates refused, withheld and missed doses to the house lead and medication admins
  • Encrypts medication and MAR details at field level

It doesn't

  • Doesn't suggest doses or give clinical advice
  • Doesn't connect to pharmacies or prescribing systems
  • Doesn't save dose records offline; recording a dose needs a connection

Frequently asked questions

Does it support PRN (as-needed) medication?

Yes, with a maximum number of doses in 24 hours and a minimum interval, a recorded reason and a follow-up on whether it worked.

Can it require a second person?

Yes. High-risk and Schedule 8 medicines need a witness when the dose is recorded: a second staff member signs in with their own email and password on the same device. High-risk medicines also need a double check.

Can workers record doses on their phone?

Yes. The mobile app shows what is due now, what is coming up and PRN medicines, with a step-by-step recording form. Workers record given, self-administered, refused, withheld, missed or not required (with a reason). Recording a dose needs a connection; doses aren't saved offline.

Does it connect to our pharmacy?

No. Medication details are entered by your team. There is no pharmacy integration.

How it works

  1. Add the medication

Record the medicine, dose, instructions, a photo and stock, and PRN rules where they apply.

  2. Record each dose

The worker on shift records the outcome: given, self-administered, refused, withheld, missed or not required (scheduled doses only, with a reason). High-risk medicines need a double check, and high-risk and Schedule 8 medicines need a witness.

  3. Get alerts

Doses with nothing recorded raise a late alert at 60 minutes and a missed alert at 3 hours.

  4. Follow up

PRN doses ask whether they worked. Refused, withheld and missed doses are escalated to the house lead and medication admins, and the app tells the worker who was told, with Call on call and Call 000.

Who uses it

  • Support workers

  • House leads

  • Clinical and medication leads

  • SIL house management

SIL house management keeps each supported independent living house with its residents, house lead and settings. Autonomy Care builds the house roster from a template, reports times when residents are home with nobody rostered, splits shared support time across residents for billing, and records handovers so the next worker sees what happened.

  • Incidents and complaints

Registered providers must manage all incidents and notify the NDIS Commission of reportable incidents within set timeframes. Autonomy Care walks workers through a report that starts with safety, tells the on-call coordinator about 24-hour categories, tracks Commission deadlines with alerts, and won't close an incident until the required steps are recorded.

  • Progress notes

A progress note records what support was delivered, what was observed and how it relates to the participant's goals. In Autonomy Care, workers write notes on the shift with short prompts, quick notes and dictation. The note coach checks each note against plain rules and current goals, and Signals flag missing notes before invoicing.
