NDIS audit checklist for providers

By Autonomy Care team. Published . Updated . 5 minute read.

An NDIS audit checks that supports were delivered safely and as agreed. This checklist covers the records auditors commonly ask for: incidents and complaints, policies with staff acknowledgements, worker screening and training, service agreements and progress notes.

Keep each progress note with the shift it describes, and keep incident records with their corrective actions.

Last checked: 14/10/2026, against the NDIS Practice Standards. From 1 July 2026, SIL providers are also audited against the new SIL Practice Standards. Your auditor confirms the standards that apply to you.

In Autonomy Care

  • Progress notes

Shift notes written on shift and read by coordinators the same day.

  • Writing progress notes that are useful the next day

A useful progress note says what support was given, how the participant responded, anything that changed or worried you, and what the next worker needs to know. Write it before you leave the shift, in plain words, with facts rather than opinions, and link it to the participant's goals where you can.
